Tax code decision table
A one-page table. Rows list item types. Columns list customer types and ship-to regions, and each cell holds the tax code, the rate and its effective date. A side block lists every code with its ledger, its invoice wording and whether it allows input tax recovery. The foot of the page names the owner, shows when the table was reviewed most recently and leaves room for the accountant’s initials, so it doubles as audit evidence.
Name: ______________________ Business: ______________________ Date: ______________
Scroll the table sideways to see every column.
