The one page worksheet has four blocks. Identity fields cover serial, work order, PCB lot and panel position, while build fields cover firmware ID, checksum, reflow date and shift. Test fields hold attempt number, station, fixture, limits version and result, and the parts block holds part number, supplier lot and date code. A final column names who captures each field.
