A one-page sheet in two halves. The top half is an order form with dealer, design, size, finish, shade, batch, quantity in boxes, rate shown and rate date. The bottom half is a dispute log with date, order number, what differed, who quoted, cause and fix, so a month of quarrels can be counted rather than remembered.
