Each activity takes one row on a single-page worksheet. Columns hold BOQ quantity, purchase order quantity, goods receipt quantity, issued quantity, recorded wastage, billed quantity and unexplained balance. A second block lists issue slips missing an activity code, and a third block lists shuttering sets with issue date and return date.
