Warranty record card
One card per order line. The top block holds the order number, the item, the reference and the serial or tag number, so one glance matches the card to the product. The middle block holds the delivery date, the wording version in force, the source invoice and the dealer authorisation letter, each as a tick box and a reference. Below sits the claim log. Each row records the date, the fault, the photographs received, the decision and the clause quoted, so a second reviewer can follow the reasoning without calling anyone.
