Do we need a mobile app or would a web application be enough?

If your users work from a desk or a fixed counter, a web application is simpler to build and update. A mobile app earns its cost when people are moving between locations, need the camera or GPS, or must keep working without network coverage.

What decides whether we need one platform or both Android and iOS?

Look at who is using it. Field staff and delivery teams on company-issued phones are usually all on one platform, most often Android, so a single build may be enough. Customer-facing apps generally need both, since you cannot predict which phone a customer owns.
