Dental Patient Records and Medical History
Each patient has one record that holds contact details, medical history, allergies, current medicines and every visit. The dentist and the front desk open the same record from different screens.
What a patient record holds
Who uses it
The front desk registers the patient and keeps the phone number current. Treating dentists read the history and add clinical notes. An assistant checks the allergy line before the chair is prepared, and each role sees only what its work needs.
How it connects to the next step
The record is where the tooth chart, treatment plan, X-rays and bills all attach. A change to the history shows to every dentist who opens the case, a visiting specialist included.
What the clinic decides
Which history questions appear on the form, and how long old records are kept, are decisions for the clinic and its own advisers. The system stores what is entered. It does not interpret a history.
Frequently asked questions
Can the history form use our own questions?
Yes. The fields on the registration and history form are set with the clinic, including any questions a dentist wants on every new patient.
Can old patient records be brought in?
Yes. Patient lists and visit summaries are imported from existing software or a register, and the receptionist checks them before the clinic starts using the system.
Is a child linked to a parent?
Yes. Family members can be linked, so reminders and balances for a child reach the parent’s number.
Who can see a patient’s medical history?
Only roles the clinic allows. The desk can be limited to contact and billing, while dentists and assistants see the clinical record.
