Dental X-Ray and Consent Records for Patients
IOPA and OPG images and signed consent forms attach to the patient record and to the tooth they concern. Any dentist who opens the case can find them without searching a folder.

Consent forms
A consent form is printed from the clinic’s own wording, signed by the patient or a parent, and scanned or photographed onto the record. Forms can be tied to a procedure, such as an extraction or an implant. Staff see at once whether one is on file.
Who uses them
Assistants take and upload the images. Dentists read them during examination and planning. The desk checks that a signed consent is attached before a procedure begins.
What the clinic decides
What each consent form says, which procedures need one and how long images are kept are for the clinic and its own advisers to settle. The system stores and shows the files. It does not read an image or judge a form.
Frequently asked questions
Does it connect to our X-ray machine?
Not by default. Images are uploaded to the record. Whether a particular sensor or software can feed files in is checked during scoping.
Can a patient sign on a screen?
Signing on a tablet can be set up where the clinic has one. Otherwise the paper form is signed and then scanned onto the record.
Can we write our own consent wording?
Yes. The forms use the clinic’s own text. What they say is decided with the clinic, not by GullySystem.
Can images be shared with a specialist?
A dentist with access can open them. Sending a file outside the clinic, and the patient’s agreement to it, is for the clinic to arrange.
