Consent and Declaration Forms

Contrast studies, certain ultrasound scans and some procedures need a signed form. The form is printed or shown on a tablet. Once signed, it is attached to the visit it belongs to.

Forms the centre can keep

The centre supplies the wording of each form.

Contrast consent

Signed before a contrast study, with the creatinine value and allergy questions noted where the form asks.

Ultrasound declarations

The declarations and registers the centre uses for its ultrasound work, filled at the visit.

Other procedures

Any further form the centre’s doctors decide to use for a study.

Finding a form later

Forms are searched by patient, study, doctor or date. A register view lists every form of one kind for a chosen period, which helps when the centre’s own staff or an inspector asks to see them together. What these records must contain, and how long they are kept, is for the centre and its adviser to confirm.

Yes, where the centre uses one. A signed paper form can also be scanned and attached.

No. The centre sets which studies need which form. The system prompts for it at the scan.
