Case Taking Forms for Homeopathy Doctors
The case form follows the headings your doctor already writes: complaints, modalities, mental and physical generals, family history and past illness. A new case is taken once, and later visits add to it.
Headings of the form
Homeopathic case taking is long and varies by doctor. The form carries your doctor’s own headings in the order they are asked.
Taking the case on a screen
The doctor can type during the consultation or note the main points and complete the form afterwards. Fields that do not apply are left empty. Children and adults can have separate forms. A paediatric case then carries no questions meant for grown patients, and the mother’s account of pregnancy and birth has its own place.
Changing the form
Doctors refine how they take a case over the years. GullySystem staff add, rename or remove headings whenever the doctor asks. Old files stay readable. Cases already recorded keep the form they were taken on.
What the doctor decides
- Which headings appear and in what order
- Which fields are compulsory for a new case
- Whether adults and children use different forms
- Whether staff may fill part of the form before the doctor
- Which fields are hidden from the front desk
Frequently asked questions
Can our own case sheet layout be copied?
Yes. The headings from your printed sheet are the starting point, and our team builds the form from it.
Does the form suggest questions to ask?
No. The doctor decides what to ask. The form only provides the places where the answers are written.
Can a nurse or assistant record vitals first?
Yes, where the clinic assigns that to the staff. Weight, pulse and blood pressure can be entered before the doctor starts.
Can the form be printed?
Yes. A completed case can be printed, and a blank form can be printed for use when a screen is not at hand.
