Patient Case Records for Homeopathy Clinics
Each patient has one file that holds details, case history, visits, prescriptions, scanned papers and photographs. The doctor opens it from the day’s queue and reads the last visit before the patient sits down.
What the patient file holds
A chronic case can run for years and pass through many prescriptions. The file keeps that whole course in order.
Families and repeat patients
Members of one family often consult the same doctor. They can be linked, so the desk finds a child’s file from the mother’s phone number. Each person keeps a separate case record, because each is prescribed for on his or her own symptoms.
Who may open which part
Case notes are private. The desk sees contact details, visits and bills. Doctors see the clinical notes. The clinic decides which roles may open or edit each part, and changes to a note are dated.
What the file does not do
The file stores what the doctor and the desk enter. It does not read handwriting from old sheets. Paper files are scanned and attached, and which old cases are worth keying in is for the doctor to decide.
Frequently asked questions
Can old paper case sheets be added?
Yes. They are scanned and attached to the patient’s file. Typing them in line by line is optional and decided by the doctor.
How does the desk find a patient quickly?
By name, phone number or file number. A family linked together appears under one phone number.
Can a photograph be attached to a visit?
Yes. Photographs of a skin, hair or nail complaint carry the date of the visit, so the doctor can compare them later.
Is each doctor’s patient list separate?
Where a clinic has several doctors, the list can be shared or kept apart. The owner chooses during set-up.
