Adjustment Visit Notes for Chiropractic Clinics
Each adjustment visit is written up in a short note: region treated, technique, the patient’s response and the advice given. The note counts the visit against the treatment plan.
What a visit note holds
A short note for a busy day
Most visits are brief. The form is built to be filled in under a minute, with choices instead of typing. A doctor with many patients can still leave a usable note.
How it connects
Saving the note adds one visit to the patient’s treatment plan. The visit then appears on the bill, and the calendar offers the next slot in the gap the doctor set.
What the note does not do
The note records what the doctor did. It does not decide the technique, and it does not alter the plan. The doctor changes a plan on the plan screen, with the reason written down.
Frequently asked questions
Can the notes be dictated?
Voice entry through the phone keyboard works as typing does. A separate dictation tool is not part of the product.
Can a patient see the visit notes?
Only if the clinic chooses to share a summary. Full notes stay with the clinic.
What if a visit was not an adjustment?
A review, a consultation or a counselling visit is marked as such, and it does not count against the visit total.
Can a note be corrected afterwards?
Yes, by the author or the owner. The change is logged with the user and the time.
