Patient Case History for Chiropractic Clinics
Each chiropractic patient has one profile holding the complaint, history, past injuries and medical conditions. The doctor reads it before the first adjustment and again at every review.
What the profile holds
Who writes and who reads
Reception enters name, age, phone number, address and the reason for the visit. The chiropractor adds the history. A second chiropractor covering a leave day opens the same profile and reads the findings before the patient is seen.
Where it leads next
The case history is the first record in a patient’s file. The spinal assessment, scans and treatment plan are all attached to it, and so is every bill.
What the clinic decides
The history fields are set up with your doctors. A clinic that asks about work posture or sleeping position can add those questions. The software stores answers and does not suggest a diagnosis.
Frequently asked questions
Can a patient’s family members be linked?
Yes. Profiles for a husband, wife or parent and child can be linked, so the front desk books them together and sees one phone number.
What happens to the old paper cards?
Active patients are keyed in or scanned at the start. Which older cards are worth adding is decided with the clinic.
Can the doctor see who edited a note?
Yes. Each entry carries the name of the user and the time it was saved.
Does it diagnose from the history?
No. It stores what the doctor records. Diagnosis stays with the chiropractor.
