Spinal and Posture Assessment for Chiropractic Clinics
The assessment form records findings by spinal region, posture and range of movement. The chiropractor fills it at the first visit and again at each review, on the same fields.
What the assessment records
Same fields at each review
A review repeats the same assessment. The doctor then sees the first visit’s values beside the new ones. A restricted range that has opened up is plain to see.
Who uses the form
Chiropractors fill and read the assessment. The front desk sees that one exists, and nothing more. An assistant who positions the patient can be given the posture section, if the clinic wants it that way.
What the clinic sets up
Your doctors choose the regions, tests and scales that appear. Two chiropractors in one clinic may use different forms if they assess differently. Interpretation stays with the doctor. Treatment is never suggested.
Frequently asked questions
Can we use our own printed assessment form?
Yes. Fields are copied from the form your doctors use today, and the clinic reviews the layout before it goes live.
Can a spine diagram be marked?
Yes, where the clinic wants it. A body or spine outline can be marked and saved with the visit. The clinic chooses this at configuration.
Are photos stored with the record?
Yes. A posture photo is attached to the patient’s profile with the date, after the patient agrees.
Can an assessment be printed for the patient?
Yes. A summary prints or goes to the patient, and the clinic picks which fields appear.
