Patient Case Sheets for Ayurveda Clinics

The case sheet holds each patient’s details, history, prakriti notes and every earlier visit in one record. Vaidyas, the front desk and the dispensary open the same sheet.

What the case sheet holds

Identity and contact

Name, age, address and phone number are entered with a guardian’s contact for children and elderly patients, and a patient who already has a record is flagged at registration.

History and allergies

Past illnesses, medicines taken from other systems, known allergies and family history are entered once and shown again at every later visit, so the vaidya starts with the full picture.

Prakriti and findings

Prakriti notes, pulse and other findings go in under the headings your vaidya uses. Each visit adds to the sheet. It never overwrites the last one.

Visit timeline

Consultations, prescriptions, therapy courses and bills sit on one timeline, and the vaidya reads the earlier visits before writing the next prescription.

Who opens the sheet

The front desk registers the patient and keeps contact details current, while vaidyas read and write the clinical notes and therapists see the therapy advised. Any caution the vaidya has marked shows on the therapist’s list. The dispensary sees the prescription it must fill. What each role may open is set with the clinic.

Where the sheet leads next

A saved consultation passes to the prescription, and from there to the dispensary and the treatment course, so no detail is entered a second time. The same sheet feeds the list of patients due for review.

What the client decides

The sheet stores what the vaidya writes. It does not diagnose and does not suggest treatment.

Can old paper case sheets be added?

Yes. Recent patients can be typed in or attached as scans. How far back to go is settled with the clinic during scoping.

Can notes be written in a regional language?

Yes, where the clinic types in that language. Printed formats and headings are set up with the clinic.

Who can change a sheet after the visit?

Only the roles the clinic allows. Each change carries the editor’s name and the time, so the history stays readable.

Does the sheet suggest a diagnosis?

No. It stores and prints what the vaidya enters. Diagnosis and treatment remain the vaidya’s decision.
