Pathologist Review and Sign-Off for Pathology Labs
No report leaves the lab until a pathologist has reviewed and signed it. The sign-off queue puts flagged and critical results at the top. Flagged values, earlier results and the technician’s notes sit on one screen, so the pathologist can decide to sign, hold or re-run without asking the bench.
The review screen
Sign, hold or send back
The pathologist signs the report, holds it for a query, or sends a test back for a re-run. A critical value can be marked as phoned to the doctor, with the time and the person spoken to. Every action carries the pathologist’s name and the time.
Amended reports
Sometimes a signed report needs a correction. The original stays on record, and the amended version is signed again. The report shows it is amended, and both versions can be traced. Each carries a signature.
Frequently asked questions
Can more than one pathologist sign?
Yes. Each signs under their own login, and the report shows who signed it.
Can sign-off be done from outside the lab?
Yes, where the lab allows remote login for its pathologist. Access is limited to that role.
Is a provisional report possible?
Yes, for long-running tests such as cultures. It is signed as provisional and followed by a final report.
