Consent Records for Skin and Aesthetics Clinics
A consent form is prepared for each procedure, signed by the patient and filed against the session. The desk can see which consents are pending before the procedure begins.
What is kept
Pending consents
A session that requires a form shows a flag until the form is signed. The therapist sees it before starting. This is a prompt, not a lock, and the clinic decides whether a flagged session may go ahead.
Repeat sessions
For a course of the same procedure, the clinic decides whether one consent covers every sitting or a fresh form is taken each time. The system follows that choice and flags accordingly.
What the clinic remains responsible for
GullySystem stores and files the forms. It does not write medical or legal wording. The text of each form, how long it is kept and what the clinic must retain are for the doctor and the clinic’s advisor to settle.
Frequently asked questions
Can we use our existing consent forms?
Yes. Your wording is entered for each procedure during set-up, or the scanned form is attached to the record.
Can a patient sign on a tablet?
Yes, where the clinic has a tablet at the desk. A printed form can be signed and scanned instead.
Does it tell us which consents are legally needed?
No. Which forms a procedure needs is for the doctor and the clinic to decide, and the system holds that choice.
Can a lost form be found by date?
Yes. Forms are filed against the patient, the procedure and the session date.
