Healthcare · Hospice Management Software

One shared record for hospice referrals, care plans, home visits, equipment and billing.

Manage a hospice service in one system. Referrals become admissions, and each patient’s care plan, visits, medicines and family contacts sit on one record. Nurses, aides, social workers, chaplains and volunteers all see it. Loaned equipment, after-hours calls and charges all come from what staff have already entered.

  • Disciplines, levels of care and visit types entered as your service works
  • Active patient list and open care plans migrated
  • Changes to forms and visit lists handled by GullySystem staff when asked

GullySystem configures this system for hospice providers, whether they run a home care team, an inpatient unit or both. Intake staff, nurses, aides, social workers, chaplains, volunteers and billing teams work from one patient record. It carries the care plan, each visit, on-call contacts, loaned equipment and the charges raised.

Where it breaks today

What goes wrong before the software.

Each discipline keeps its own notes

Nursing, aide, social work and spiritual care notes sit in separate files. A team meeting begins with someone gathering them.

Referrals are handled on the phone

A call arrives from a hospital ward or a family. Who took it, what was promised and when the first visit is due are written on whatever was nearby.

Care plans change faster than the paper

Medicines and symptom plans are adjusted often. The version in the home, the office and the on-call phone can all differ.

Visits are planned from memory

Which patients need a visit today and who is nearest are decided by a coordinator’s knowledge. When that person is absent, the plan is lost.

Equipment moves without a record

A hospital bed or an oxygen unit goes to a home and a return is expected. The list is a spreadsheet nobody updates the same day.

Family follow-up has no owner

Bereavement calls and letters depend on one coordinator’s diary. After a death, the record usually stops being opened.

How it runs

How the work flows through it, step by step.

  1. 1

    Referral

Intake logs the source, diagnosis, address and main caregiver, and the case is passed to a nurse for assessment.

  2. 2

    Admission

The admission visit is completed, consent is recorded, and the medical director’s certification and first orders are attached.

  3. 3

    Care plan

Each discipline adds its goals to the plan. Medicines, symptom guidance and the patient’s stated wishes are visible to the whole team.

  4. 4

    Visits and on-call

Visits are assigned by area and need. After hours, the on-call nurse opens the same record and logs the call.

  5. 5

    Team review

The interdisciplinary team reviews each patient on its schedule, and actions are given to named people with dates.

  6. 6

    Billing and follow-up

Charges are raised from the recorded visits at the right level of care, and family follow-up is scheduled after a death.

Patient and family record

Hold the patient’s history, diagnosis, medicines, allergies, advance wishes and the family and caregiver contacts on one record.

Admission, consent and certification

Record the admission visit, signed consent and certification, with the dates each must be renewed.

Care plans by discipline

Keep goals and instructions for nursing, aide care, social work and spiritual care, with changes dated and attributed.

Visit scheduling

Assign visits by area, discipline and patient need, with route order, visit notes and missed visits flagged.

Medicines and comfort kits

On-call and symptom log

Give the on-call nurse the current plan and log each after-hours call, advice given and visit sent.

Equipment loans

Track beds, mattresses, oxygen units and commodes by patient, location, delivery, inspection and return.

Interdisciplinary team reviews

Prepare the review list, record decisions for each patient, and carry the actions forward to the next meeting.

Volunteers and visit hours

Match volunteers to patients, record the hours given and keep training and clearance dates in one place.

Billing by level of care

Raise charges from the visits recorded, with level of care, signed orders and payer details attached.

Bereavement support

Schedule calls, letters and group invitations for families, and note who has accepted support.

Solutions inside

The business modules it is built from.

Each one is also available on its own, or inside a system built for you.

  • Technician Scheduling

The day’s nurse, aide and social work visits are placed against staff by area and need, and missed visits are flagged to the coordinator.

  • Document Management

Signed consent, certification and first orders are stored on the patient record, each with the date it must be renewed.

  • Fixed Asset Register

Every bed, mattress and oxygen unit has its own entry showing which home it is in, when it was delivered and when it was last inspected.

  • Stock Tracking and Alerts

Comfort kits and base stock are counted by item and batch, and a low or expiring item is flagged before a visit needs it.

  • Service Reminders

After a death, calls, letters and group invitations are scheduled for the family and appear on the coordinator’s list when due.

  • Workflow and Approvals

A referral moves through assessment, acceptance and admission, with the clinician who must approve each step named on the record.

  • Invoice Management

Charges come from visits already recorded, grouped by level of care and payer, so billing staff do not retype notes.

  • Roles and Permissions

Sensitive notes are shown by role, so an aide sees the care tasks while clinicians see history, medicines and wishes.

Services

How we would put it in.

Setup, data migration, integrations and anything built on top.

Who uses it

One system, each person sees their part.

For the clinical director

See admissions, patient mix, visits completed, reviews due and recertification dates across every team.

For nurse case managers

Read your patients’ plans, plan the visit round, record notes and update medicines from one list.

For aides and volunteers

See the visits you are assigned, the patient’s preferences and the tasks to be done, and leave a short note.

For social workers and chaplains

Record family needs, practical arrangements and spiritual care, and see what the rest of the team has noted.

In billing and administration

Check visits against orders, raise charges, follow payer queries and keep the equipment list current.

Reports

The numbers the owner asks for.

  • Referrals received, admitted and declined
  • Visits planned, completed and missed by discipline
  • Patients with certification or review dates coming due
  • Equipment on loan by patient and inspection date
  • After-hours calls by reason and outcome
  • Charges raised by level of care and payer
  • Bereavement follow-ups due and completed
Good fit

Who this is built for.

  • Home-based hospice teams visiting patients across a region
  • Hospices with an inpatient unit or respite beds
  • Providers that also run palliative care consultations
  • Charitable hospices that rely on volunteers and donated equipment
  • Hospice groups with more than one base or branch
Honest answer

When it is not the right choice.

  • A single consultant who sees a few palliative patients can keep a diary and a clinic record.
  • Hospital/Clinic Management System suits a general hospital better, with its wards, theatres and pharmacy.
Made to fit

Adapted to your business before it goes live.

You are not handed a login and left to it. The product is set up around your records, your people and the tools you already use.

Disciplines and care levels

Staff roles, visit types and levels of care carry the names your service uses on its rotas and bills.

Forms and care plan layout

Your own admission forms, consent wording and plan sections are used. Their content is decided with your clinical leaders and advisers.

Patients and equipment migrated

Active patients, open plans and the equipment list arrive from your existing record, spreadsheets or paper files.

Language and billing format

The languages shown to staff and families, the tax lines and the currency follow your country, and are reviewed at implementation.

Where it runs

The system runs on GullySystem servers, so each base works from one patient list. Edits to forms and lists are made by GullySystem staff.

Common questions

Questions owners ask before a demo.

Does it replace our clinical record?

It can hold the patient record, plans, visits and orders. Whether it replaces an existing clinical system or sits beside it is decided during implementation.

How does it serve nurses on home visits?

Yes. Nurses and aides open the day’s visit list, the care plan and the visit note on a handheld device in the home. Connectivity in the field varies, so offline needs are reviewed with you.

How does the on-call nurse see the current plan?

The same patient record opens after hours. The on-call view shows medicines, the symptom plan, recent calls and the family contact first.

Can it track volunteers as well as staff?

Yes. Volunteers have a profile with their training, clearance dates, assigned patients and the hours they have given.

Does it handle equipment on loan?

Yes. Each bed, mattress or oxygen unit is recorded against a patient and a place, with inspection dates and the return logged.

Can it prepare claims for our payers?

It prepares charges from recorded visits and attaches the orders. Submission formats differ by payer and country, so a direct link is reviewed and quoted separately.

Can our existing patient list come with us?

Yes. Current patients, open care plans and family contacts are brought in from a file or an export, and clinical staff review the list before the first day of use.

Whose property are the patient records?

The provider’s. Export format and retention are agreed when the proposal is written, with local privacy rules reviewed alongside your advisers.

What does it cost?

No price is published. Team and user numbers, plus how much set-up the service needs, shape the figure. We show a demonstration, then give a quotation.

Wondering what it costs? Six questions give you a written scope and the buckets any quote should separate.
